Indications for Non-Invasive Ventilation (NIV)

Mnemonic: “COPD CAN Breathe”

  • COPD exacerbation with hypercapnia and respiratory acidosis.
  • Cardiogenic pulmonary oedema with respiratory distress.
  • Chronic respiratory failure (neuromuscular diseases, obesity hypoventilation syndrome, chest wall deformities).
  • Acute hypoxemic respiratory failure (selected patients, PaO₂/FiO₂ < 200).
  • Non-invasive support post-extubation to prevent reintubation.
  • Breathe (palliative care with DNI orders to ease dyspnea).

Contraindications for Non-Invasive Ventilation (NIV)

Mnemonic: “CRASH FACE”

  1. Cardiac or respiratory arrest.
  2. Risk of aspiration due to inability to protect the airway.
  3. Active vomiting or copious secretions.
  4. Severe facial trauma, burns, or structural abnormalities.
  5. Hemodynamic instability or life-threatening arrhythmias.

FACE:

  • Failed mask fit due to anatomy or burns.
  • Agitation or inability to cooperate.
  • Critical acidosis (pH < 7.15).
  • Extreme hypoxemia requiring invasive ventilation.
